The short version: a multi-location specialty healthcare practice grew from 26,683 organic visitors in its first year with COUR Digital to 650,845 in year six, which is roughly 24 times the starting point. Organic search now brings in the large majority of its new patient inquiries.

The growth rate slows as the base gets larger, which is what you would expect, because adding 93,000 visitors to a site that already sees more than half a million is harder work than tripling a small one. What matters to the practice is that organic traffic grew in every one of the six years.

Where new patients came from

Traffic is only useful if it turns into inquiries, so we tracked conversions by channel. In the primary market, organic search produced 66.9% of the 411 screening requests and 70% of the 270 contact form submissions. In the practice’s second country, where the site launched with no traffic history, organic search reached 80,424 annual visitors and produced 73.5% of local inquiries.

More than 90% of search traffic came from non-branded queries, meaning people who had not heard of the practice before they searched for help with a specific concern and found its content first.

What drove the results

  • Clinician-informed content. The top five articles drew between 27,000 and 81,000 pageviews each in a single year, and every one of them was built from what the clinical team explains to patients every day.
  • A technical foundation that scaled. Site architecture that prevented specialty pages from competing with each other, structured internal linking, healthcare schema, and Core Web Vitals work.
  • Local SEO for each location. Every clinic received its own Google Business Profile optimization and a location page written for that community, rather than a city name swapped into a template.
  • Google Ads in a supporting role. Paid search covered priority services while organic rankings were still building, and budget shifted as those rankings took hold.

What this means for your practice

The $414,000 figure is Semrush’s estimate of what it would cost each year to buy the same keyword visibility through Google Ads. Paid traffic stops when the budget stops, while the organic visibility this practice built keeps producing inquiries, which is why we treat SEO as an asset on the balance sheet rather than a monthly expense.
